Intake Form

To speed up the intake process on your first visit, you can now download and print off our intake form and complete this before coming in for your first treatment.

Consent Form

Written consent to treatment must be provided prior to your first treatment. Your therapist will also discuss what you can expect from your treatments and provide a projected treatment plan at this time.

If you cannot print off these forms, please arrive 15 minutes early for your first treatment, to ensure adequate time for intake.

Please remember to wear loose, comfortable clothing for your treatment!
